  • Which actions need approval?
  • Where are prompts, decisions, and outputs logged?
  • What happens when confidence is low?
  • Who reviews exceptions?
  • Can the system be paused quickly?
  • Can a human reproduce the reasoning path?

Human-in-the-loop does not mean a person clicks every button. It means the system knows when human judgment matters and makes that moment explicit.
